Are you using a mobile device or a PC? And if it's PC, I know some browsers can be very naughty at hiding the averages of your players, but not their positions.

If you could, you can take a screen shot of what you are seeing on your screen.

 Not sure what you are trying to set. Whether basic formation or advanced one. Or whether you are trying to setup lineup or a formation. Screenshot and more info (device, browser, versions, what you are trying to do) would definitely help.

As a quick fix, anytime I have a problem setting up formation or lineup, I click "Automatic" button in lineup screen then turn it off and adjust manually. Sometimes, when you sell player or move from juniors to seniors and vice versa (returning from temp. promotion) while you are having them in the starting eleven, they disappear and the hole which remains in a lineup cannot be filled up on certain devices.
