I see this issue frequently in my recent league game where 2 opposing players get stuck either at the ball or away from the ball and they both just keep falling over until a foul is called. Sometimes they can fall over 10 or more times in a row before a foul is called. Examples from the following match: http://uk.strikermanager.com/partido.php?id=3928874&accion=simul 11:30 - Stuck on the ball, 8 duel falls in a row then a penalty. 20:10 - Players stuck away from ball and continue to fall repeatedly until 21:36 or so when play stops.
